POINTS TO BE REMEMBERED WHILE SUBMITTING NEW PROPOSALS

  1. All Non-Medical cases should be STRICTLY completed under ‘GREEN CHANNEL only.
  2. Age Proof Clarifications
    i) Wherever NSAP I. II or lii is submitted, a declaration in Form 3260 is to be obtained from the client, agent and Dev. Officer / ABM (5). Form 3260 which do not bear the signatures of the specified officials will not be valid.

    ii) Ration cards issued originally, clearly mentioning the date of issue will only be accepted as age-proof under NSAP III (Class I extra to be charged). Ration cards where date of issue is not available will not be accepted as age-proof.

    (iii) Horoscopes which are produced only for the purpose of insurance (single page computer horoscope) will not be accepted as age-proof).

    (iv) Self attestation of the proposer in the age-proof is compulsory

    (v) Wherever the age proof is previous policy which pertains to other Branches previous policy extract is a must. (Status reports Will not be accepted for age-proof)

    (vi) For age-proof NSAP I, II & III maximum term 26 years and maximum age at entry 50 years and maximum maturity age 65 years respectively
  3. MHR Clarifications
    (ii) Proposals on Jeevan Sathi Plan
    (iii) Proposals on the lives of Deaf and Dumb stating that they are gainfully employed and mentally sound.
    (iv) Proposals on all category Ill lady lives by agents below DM’s Club.
    (v) Proposals on all NMG cases where ACR is given by
    a) non-club member agents
    b) BM club members
    (vi) Agents / Officials giving ACR / MHR should clearly write their names and also specify their date of appointment.
  4. Table Nos. 43. 52, 58 and 164 will not be allowed under Non-Medical
  5. CIR will not be allowed under Non-medical and also sub-standard life.
  6. Insurance cover will be allowed to married women falling under category Ill if they have their own earned or unearned income (not filing income tax returns) over and above insurance cover on the basis of their husband’s income / insurance.
    Rs. 1 lakh if illiterate or educated upto 8th standard and Rs. 5 Iakh if educated upto 9th or above.
  7. Table 91 loses its special status. Use of Form 320 (JP) is discontinued. Proposals are to be submitted in Form 300 along with age proof only
  8. For hand delivery of policies, please obtain an acknowledgment letter along with the proposal itself.
